IMPORTANT NOTICE

Your Affordable Care Act (ACA) Form 1095-C is available upon request.

Under the Paperwork Burden Reduction Act, HireCall is no longer required to automatically mail a Form 1095-C to all employees. However, this form, which includes important information about the health coverage offered to you, is available upon request.

How to request your Form 1095-C for the 2025 tax year:

After you submit your request:
Upon receiving your request, we will provide your Form 1095-C within 30 days. The form will be sent to the address we have on file for you. If you have provided consent for electronic delivery, it will be sent to you electronically.

This notice will remain on our website through at least October 15, 2026.

For federal purposes, you do not need to submit the Form 1095-C with your tax return.
